I made this questionnaire you can use for prospective donore. As with everything I feel the Vampire and Donor Relationship must be safe.

1). Do you have any history of blood disease, hemophilia, or any condition that required blood thinners? Have you been in contact with any products that had Vitamin K?

2). Have you taken anything like Pseudophed, in tha past 3 days?

3). Have you had any dental work on the past 3 weeks?

4) Have you failed an STD test in the past year? Taken meds for an STD?

5) Do you have a history of seizure, anaphilactic shock or heart conditions? Are you taking a heart medicine or something for any cardiac condition?

These questions are very important. BE SAFE!!!!
