Extremely drug-resistant TB found in U.S.; weaker form spreads globally

Public health officials are concerned about the implications of the first U.S. case of extremely drug-resistant tuberculosis, diagnosed in a 19-year-old Peruvian who is visiting here to study English, Homeland Security Today reported Dec. 29.
Oswaldo Juarez contracted the extremely drug-resistant XXDR-TB in 2007 without having had tuberculosis before. This form of drug-resistant TB is virtually impervious to all the drugs that are used to treat the disease.
Elsewhere, scientists have identified a strain of antibiotic-resistant tuberculosis that is resistant to rifampin, the front-line antibiotic to treat TB. The rifampin-resistant strain was identified in a patient in China and is described in a study that will appear in the January issue of The International Journal of Tuberculosis and Lung Disease.
The World Health Organization says multidrug-resistant TB (MDR-TB) is becoming especially dangerous because many patients do not adhere to their six-month TB treatment regimen.
The WHO reported, “TB can usually be treated with a course of four standard, or first-line, anti-TB drugs. If these are misused or mismanaged [that is, when drugs are taken in the wrong combination, are fewer than those prescribed, or are taken in insufficient doses or not at the proper time], multidrug-resistant TB can develop.”
And “if these drugs are also misused or mismanaged, extensively drug-resistant TB [XDR-TB] can develop.”
“There is probably no difference between the speed of transmission of MDR-TB or XDR-TB and those of any other forms of TB,” the WHO said. “The spread of TB bacteria depends on factors such as the number and concentration of infectious people in any one place and the time of exposure, along with the presence of people with a higher risk of being infected, such as those with HIV.”
Public health officials have been watching for cases of TB during the H1N1 influenza pandemic because of concerns that people most vulnerable to the pandemic strain of flu could also more easily be infected with TB because of their compromised immune system.
XDR-TB is still a relatively rare form of MDR-TB, but among those co-infected with HIV, the mortality rate is about 90 percent. XDR-TB alarms public health officials because the disease is transmitted, like other forms of TB, through coughs or sneezes of infected individuals.
The XXDR strain of TB is so rare that only a handful of other people in the world are thought to have had it. The strain is resistant to all tuberculosis drugs and is almost always fatal.
HSToday reported more than a year ago that while new cases of TB in the United States declined sharply during the 15 years up until 2007, there are growing concerns over the sudden escalation of the disease, especially the drug-resistant forms, some of which are highly contagious airborne strains of the disease.
“We should continue to fund AIDS vaccines and research into more effective antiretroviral medicines, but these are long-term projects,” said Dr. David Miller, an outspoken advocate for devoting research dollars to the fight against tuberculosis and an internationally known AIDS treatment activist,
"Today, more people with HIV are dying from tuberculosis, a disease with the potential to spread like wildfire in a crowded urban environment," Miller said in a statement Dec. 29.
Dr. David Ashkin, one of the nation's leading experts on tuberculosis, told The Associated Press that XXDR “is really the future. This is the new class that people are not really talking too much about. These are the ones we really fear because I'm not sure how we treat them."
Juarez spent most of the two years after his diagnosis at a quarantine hospital in Florida.
"When he first came in we really had to throw everything and the kitchen sink at him," said Ashkin, the hospital's medical director, who experimented on Juarez with high doses of drugs, some not typically used for TB. "It was definitely cutting edge and definitely somewhat risky because it's not like I can go to the textbooks or ... journal articles to find out how to do this."
The treatments worked. Juarez, by then 21 years old, left the hospital in July.

Seeing the look on my daughter's eyes this morning was priceless. She looked at me and said, "Are those mine?" I replied, "Yes, they are, Santa brought them for you." She was so excited.

Why do people feel they need to shoot a pit bull?
Pit bulls are not bad dogs. The people that raise them to be fighters are the bad ones. It is not the dog. By nature they are very loving animals. Shoot the people who breed them and train them to be aggressive and fighters.
For example, Michael Vick. He should have been treated the way he treated the poor dogs.
Take a moment and watch the hour long show on the Discovery Channel called Pit Bulls and Parolees. It is a great show and shows just how loving the dogs can be.

Those damn people did not get a harsh sentence. They got misdemeanors. How in the hell is that possible? The American legal systems needs to be a lot better.
They didn't even go to jail, got freaking probation.
These children deserve to be given back to their families.
It may be really hard on the families that adopted them, but think about the family that gave them up. Thinking that it was a program to get a better education and be able to come back home.
Damn those people for taking advantage of poor uneducated people to make a damn dollar.

I am watching a program on 48 Hours called the Lost Children.
People were running a scam on adopting Samoan children. The people should be shot for taking these children from their Samoan families and letting the adopting families think the children were theirs.

Our community is doing an Adopt - A - Kid or Adopt - A - Senior this year for Christmas. On Tuesday I went to the local radio station and adopted a 17 year old boy and a 16 year old girl. When I was there, they told me a local business came in and got 20 names. They said they still had like 60 or 65 left. I wish I could buy for them all. But money is a little tight. So I figured I could at least two kids.
I couldn't believe what these kids and all the other ones were asking for Christmas. They asked for basic essentials.

So today I went to the store and bought what they asked for. It made me feel so good to be able to brighten a couple of the kids in my community. I hope that they have a great Christmas now.
If I have enough money left over after the grocery store tomorrow, I will go the local YMCA and adopt a senior.
I think this is a good thing that the community does.
I am so tired of hearing about Tiger Woods' infidelity. Who gives a shit. We have presidents who do and many politicians (and they run the country) that didn't get this much publicity of it.
Move on with your lives people. People cheat on their significant others all the time. It is THEIR life, let them deal with it.

RISING SUN, Ind. (AP) — Showing no emotion and with his parents a few blocks from the courthouse, an Indiana teenager pleaded not guilty to killing his 10-year-old brother. His mother and father were greeting mourners who came to pay respects to their slain younger son.
Andrew Conley, 17, was charged as an adult in Ohio Circuit Court with murdering his brother, Conner Conley, a decision that the boys' parents supported, said Dearborn-Ohio County Prosecutor Aaron Negangard. In front of a few of his friends, the teen answered only basic questions during Friday's hearing. His attorney, Gary Sorge, entered a not guilty plea on his behalf, and left the courthouse without talking to reporters.
Nearby, the boys' parents, Shawn and Bridget Conley, greeted a steady stream of friends and well-wishers who came to a viewing of Connor's body at the Markland Funeral Home. The fifth-grader was dressed in a dark suit and lay in an open casket.
The boy's killing has left residents of this small Ohio River town 90 miles southeast of Indianapolis shaken and confused.
According to prosecutors, Andrew Conley, told investigators he strangled Conner while they were wrestling at their home Sunday, then drove his body to a park and dumped it. They say he said he did it to satisfy his craving to kill.
Townspeople described Andrew on Friday as "nice," "normal," and "polite." A relative of the family, Debbie Snyder, said the Conleys were a "strong" and "balanced" family.
And school officials said the brothers were good students and had many friends. Andrew got As and Bs, was a member of the Spanish club and had no record of disciplinary problems.
The Conleys withdrew Andrew from school on Nov. 16, 12 days before Conner's body was found near a city park, Rising Sun-Ohio County Community Schools Superintendent Stephen Patz said Friday. He declined to say why, and the parents have not talked to the media.
Prosecutors, in an affidavit, said Andrew Conley showed no remorse when he entered the police station Sunday and announced that he had killed his brother. They said he described how he choked his younger brother, saying he strangled the boy to satisfy a craving like a hungry person eating a hamburger.
They also said Conley told investigators that killing his brother made him feel like the fictional television serial killer "Dexter," and that he had cut himself in the past.
Dr. Lisa Boesky, a San Diego child psychologist and expert on teen and child behavior, said the boy's alleged self-mutilation indicated he may have been suffering from depression or anxiety, or experiencing overwhelming emotions such as rage.
Boesky said teens who kill nearly always have deep, underlying problems and that it was unlikely the television show influenced Conley's actions.
"It's rarely something that happens overnight," she said. "This is not caused by a television show or a fight with their parents. This is typically indicative of a serious disturbance. And clearly in cases like this, it indicates that something is seriously wrong."
A white cross stood Friday at the site in the park near where Connor's body was found with the messages "In loving memory of" and "classmates of Conner 2009."
Connor's great-uncle, Al Snyder, attended the visitation Friday and described the boy as outgoing, artistic and articulate.
"He could carry on a conversation better than most adults I know," Snyder, 48, of Camden, N.Y., told The Cincinnati Enquirer.
Pastor Jim Jenkins of the First Baptist Church of Rising Sun is among the clergy helping counsel students and staff at the two schools. He said the case has been especially hard on Conner's fifth-grade classmates.
"You've got a class of 20 children and all of a sudden one of them is not there and they've been murdered and their face is all over TV and the story is all over the radio," he said.
Patz, the school superintendent, said it was impossible to make sense of the crime.
"Everyone is just trying to put the pieces of the puzzle together and they aren't fitting," Patz said. "They may never fit."

Police: Va. student's rifle jammed after 2 shots

WOODBRIDGE, Va. (AP) — A community college student was upset about his grades when he walked into a classroom and fired two shots at his professor before his new rifle jammed, police said Wednesday.
Jason M. Hamilton, 20, was unable to continue shooting at mathematics professor Tatyana Kravchuk, who ducked behind a desk and was not hit, Prince William County police Maj. Ray Colgan said. No one was injured.
"Probably what prevented a further tragedy was that the gun jammed," Colgan said.
Hamilton bought the Marlin .30-06 bolt-action rifle Monday at a Dick's Sporting Goods store near the campus, police said.
Hamilton was arraigned Wednesday on charges of attempted murder and discharging a firearm in a school zone. He was being held without bond, and a preliminary hearing was scheduled for Jan. 11.
David R. Daugherty, Hamilton's attorney, declined to comment on the charges and said his client's family is asking for privacy.
"He has two parents that love him, and obviously they're going through a tremendous ordeal," said Daugherty, who was hired by the family.
Tuesday afternoon's shooting caused students and professors to scramble for cover at Northern Virginia Community College's Woodbridge campus, about 25 miles south of Washington. No one was injured in the attack.
According to police, Hamilton walked into the classroom, pulled the rifle out of a bag and pointed it at Kravchuk. He missed with the first shot, at which point she dropped behind a desk and told students to leave the room, Colgan said.
Hamilton fired again and missed again, then dropped the gun on the floor after it jammed, Colgan said. He left the room, sat in a chair and waited for police. When officers arrived, he surrendered peacefully and confessed to the shooting, Colgan said.
Colgan praised the quick response of campus police, who had participated in a training exercise Sunday on how to deal with a school shooter.
Kravchuk, 58, an assistant professor, could not be reached for comment. She received her Ph.D. from the Altai Polytechnic Institute in Russia, according to the college's Web site.
The large house where Hamilton lives with his parents is in a new development in Prince William County, an outer suburb of Washington, and appeared empty Wednesday. An Audi sedan sat in the driveway, and whimsical holiday decorations, including a reindeer and a penguin, dotted the lawn.
Jarrod Zong, a former classmate of Hamilton's at C.D. Hylton High School in Woodbridge, rang the bell and left a poinsettia for the family on the front stoop. Zong, 19, said he was on the cross-country team with Hamilton, and that Hamilton appeared to have few friends apart from the team.
"He was definitely one of the nicest people," Zong said. "Other than the cross-country team, a lot of people didn't talk to him. The team was like his family."
Zong said he hadn't been in touch with Hamilton since they graduated.
No classes were held Wednesday at the Woodbridge campus, but students and staff were allowed to retrieve their belongings. People who were in the building at the time of the shooting described the panic that ensued when shots were heard and said they thought of the 2007 Virginia Tech massacre.
Cesar Ochoa, 18, said he was in an adjacent classroom. He and several other students barricaded the door with tables, turned off the lights and huddled at the back of the room for about 25 minutes until police came, Ochoa said.
Dipak Roy, 58, an adjunct economics instructor, said he didn't realize what had happened until he went to the cafeteria and found it empty. A person in the hallway told him police had arrested a gunman but were still looking for others.
Wary of being mistaken for a shooter, Roy said he made his way back to his office and sat behind a desk with the lights off. Police eventually confronted him with assault weapons, and he slowly put his hands up.
Roy said he was wary of disgruntled students.
"We were joking about it with other faculty members: Maybe all the students deserve an A," he said.

Mo. teen charged with murder seeks to move trial

JEFFERSON CITY, Mo. (AP) — A Missouri teenager charged with killing a 9-year-old neighbor cannot get a fair trial in her home county because residents are biased against her, the teen's attorney said.
Alyssa Bustamante, 15, is to be tried in Cole County on charges of first-degree murder and armed criminal action in the Oct. 21 slaying of Elizabeth Olten. But her attorney wants the trial to be moved.
"The inhabitants of this county are so prejudiced against the defendant that a fair trial cannot be had in this county," public defender Jan King wrote in a change of venue motion dated Tuesday.
The Cole County prosecutor's office did not immediately return a phone call Wednesday. But Prosecutor Mark Richardson said last month that he would like Bustamante to be tried in the county seat of Jefferson City, which also is the state capital.
Bustamante has pleaded not guilty to Elizabeth's killing. But authorities say Bustamante confessed in a police interview to slaying Elizabeth because she wanted to know what it felt like to kill someone. They contend Bustamante strangled Elizabeth without provocation, stabbed her and cut her throat.
Hundreds of volunteers participated in a two-day search after Elizabeth disappeared before authorities say Bustamante led them to the fourth-grader's body in a wooded area near St. Martins, where both girls lived. The small town is just west of Jefferson City.
King cited the publicity generated both at the time of the crime and at Bustamante's indictment last month while asking Cole County Circuit Judge Patricia Joyce to transfer the case. He attached copies of online public comments made in response to news stories posted by the Jefferson City News Tribune and the New York Daily News.
Some of those comments refer to Bustamante as a "monster" and "inherently evil" and suggest she should be locked up for life. But it is not clear whether the people posting those comments live in Cole County, from where the jury pool normally would be drawn.
Besides allowing a case to be transferred to another county, Missouri law also allows a case to be tried in the county where the crime occurred with jurors who are brought in from other counties.
"Our belief is that the judge will make a decision that hopefully takes into account the demands upon the witnesses and victim's family," Richardson said last month.

I know everyone in the country is dealing with or having their own tough financial crisis, well I am now having mine.
Some how I have overlooked am automatic withdrawal and my account is way over the limit. I do not know how in the world I missed this, but I have really gotten myself into a pickle. I don't get paid for a week and a half and don't have enough in savings to cover it.
I tried to transfer some of my savings over to help with it a little, but the bank won't let me do the transfer.
I guess this is definitely a lesson learned.
I guess I should be thankful, that I have all my Christmas shopping done.

Glad you have too...and yea I had one do that awhile back and OMG...it was not just the one that was negative it set off 5 others that were for small amounts under 10 bucks because they were still pending and the one that was the big amount they put first so all the others even though the money was there for them counted as an extra $35 per transaction...
